Fight or Flight
Every alarming sensation in anxiety has a biological purpose. Understanding what each symptom is for transforms the experience of having it.
Three Survival Responses
When the threat response activates, the body prepares for one of three possible responses: fight, flight, or freeze. Each represents a different survival strategy depending on the nature of the perceived threat.
Fight — confront and overcome the threat. Flight — escape from the threat. Freeze — become still and undetected, or conserve energy to recover from injury. These responses are not chosen consciously. The nervous system selects automatically based on factors including the nature of the threat, past experience, and the physiological state of the organism.
In human anxiety, elements of all three can appear. Panic attacks often involve fight-or-flight characteristics (extreme arousal, urgency to escape). Some anxiety states involve a freeze quality — a paralysis, dissociation, or inability to act.
What Fight or Flight Physically Involves
The physical changes of fight or flight are specific and purposeful. Understanding each one removes much of the fear that surrounds them.
Increased heart rate — The heart pumps harder and faster to deliver oxygenated blood to the large muscles. This is preparation for physical action. It is not a heart attack.
Rapid or irregular breathing — Oxygen intake increases, and carbon dioxide is expelled more rapidly. Breathing may feel insufficient even though oxygen levels are normal. This is hyperventilation, and it produces many of the most alarming anxiety sensations.
Dizziness or light-headedness — Blood is redirected away from the head and towards the limbs. Less blood reaching the brain causes dizziness. This is temporary and not dangerous.
Chest tightness — The respiratory muscles tense in preparation for exertion. This is the same mechanism as the ache felt after intense exercise.
Tingling or numbness — Hyperventilation changes the balance of oxygen and carbon dioxide in the blood, producing tingling sensations, particularly in the hands, feet, and face.
Nausea or digestive discomfort — Digestion is suspended during emergencies. The stomach and intestines cease normal activity, producing discomfort and nausea.
Sweating — The body begins cooling itself in advance of the physical exertion of fighting or running.
Trembling or shaking — Muscles prime themselves for immediate action, producing fine tremor.
Derealisation or depersonalisation — Perceptual narrowing focuses attention intensely on the threat. The peripheral environment can feel distant, unreal, or dreamlike. This is a normal perceptual shift under extreme stress.
Why Symptoms Feel Alarming
The reason anxiety symptoms feel frightening is that they overlap substantially with the symptoms of genuine medical emergencies. Chest tightness and racing heart are also symptoms of cardiac events. Dizziness and numbness accompany some neurological conditions. The body is producing sensations that the mind — trained to associate these sensations with danger — naturally interprets as dangerous.
This misinterpretation triggers a secondary threat response. The person becomes frightened of the symptoms themselves. This fear amplifies the original response, intensifying the sensations, which then appear more alarming — a feedback loop known as the anxiety cycle.
The critical fact is that in the context of anxiety, these sensations are always benign. They are generated by the stress response system and are not indicative of any underlying physical illness. Thousands of people have presented to emergency departments with apparent cardiac or neurological emergencies, only to find — after comprehensive testing — that nothing is wrong. This is not unusual. It is the expected outcome when the fear response is responsible.
The Freeze Response
The freeze response is less discussed than fight or flight but equally significant. When neither fighting nor fleeing appears viable — when the threat feels inescapable, or when previous experiences of similar situations have been overwhelming — the nervous system may shift into a shutdown or dissociative state.
In animals, this manifests as apparent death — playing dead to discourage predators. In humans, it can appear as emotional numbness, inability to speak or act, dissociation, or a profound loss of motivation. This is not weakness. It is a survival response that served our ancestors in situations where action was impossible or counterproductive.
Recognising the freeze response is important because it is often misinterpreted as depression, laziness, or character failure. In many cases, it is simply a nervous system in a different mode of self-protection.
When There Is No Real Danger
The full sophistication of fight or flight becomes a problem when it activates without a real physical threat. The body produces an emergency-grade response to a thought, a sensation, or a social situation. The physiological reality of the experience is identical to genuine danger — only the external circumstances are not dangerous.
This is the core experience of an anxiety disorder. It is not imagination. It is not overreaction. It is a real biological event — the right machinery firing in the wrong situation. The discomfort is genuine. The danger is not.
None of These Sensations Are Dangerous
Anxiety is disordered fear — the right biological system, producing the right response, in the wrong situation. Every physical sensation it generates is the output of a functioning emergency system. No one has ever been harmed by the sensations of anxiety itself. The discomfort is real. The danger is not.
